Memberships require a 30-day written notice for cancellation. No refunds will be issued for unused benefits or partial months.
Monthly treatments do not roll over. If a treatment is missed, it is forfeited for that month.
I understand my card information will be securely stored for recurring billing through the payment processor utilized by Skin Konfidence Aesthetics and Wellness.
Membership benefits and pricing are subject to change with 30-day notice.
I authorize Skin Konfidence Aesthetics and Wellness to charge my card automatically each month until I provide written cancellation notice.
I understand this agreement will renew monthly until canceled in writing.
